The Avon Breast Health Outreach Program (BHOP) provides grants to non-profit organizations to conduct outreach, provide education on breast health and breast cancer screening, link medically underserved women to health care providers and clinical screening services, and work to ensure that they obtain any necessary follow-up care. Small community-based organizations are encouraged to apply. All applicants must register and complete an application online. All application materials and required attachments must be submitted electronically on the GIFTS grant application system.
When accessing the online grant application system for the first time you will be prompted to create an account using your email address as the User ID and a password of your choosing. You must use your functional work email address as your log-in email, as that is the address that will be recorded in the system to receive email correspondence. Completed applications and all attachments must be submitted via the online system, on or before the deadline. Extensions will not be granted, and incomplete applications or those received after the deadline will not be considered. You will receive a confirmation email shortly after you submit your application to the online system.
The Organizational Profile form does not need to be completed until you have been approved for funding.
